  • Prepare the bio-data with these columns: Ser No, Rank & Name, Address, Contact No & E-mail ID, DOB & Age, Med Cat, Domicile, Education (Civil and Cmptr), Matric with Punjabi (year), Experience.
  • Attach a copy of the Record of Service, the PPO and a photograph.
